To expedite the registration process, please print the following forms and bring them completed to the office:
- Emergency Form
- Home/School/Compact
- EUSD Student Technology Acceptable Use Agreement and Release of District from Liability
- EUSD Student/Parent/Guardian Device Agreement for 2026-2027 School Year
- Anti-Bullying Commitment 2026-2027
- Housing Questionnaire
- Children and Youth Behavioral Health Initiative (CYBHI)
In addition to these forms, please bring the following:
- Birth Certificate
- Immunization Records
- Student Transcript from previous school
- Proof of Residence
